NGMC Expands Cancer Patient Navigation Services into Braselton and Winder

Northeast Georgia Medical Center’s (NGMC) Cancer Services recently expanded its Cancer Patient Navigation Program into the Greater Braselton and Winder communities.

As a cancer patient, navigating the healthcare system with different doctors, facilities, tests and decisions can be a confusing and intimidating process. To help address these challenges, NGMC’s Cancer Patient Navigation Program offers patients guidance and support from cancer diagnosis through recovery.

Sandi Clark, BSN, RN, joins NGMC’s already robust patient navigation team of three cancer nurse navigators and one American Cancer Society navigator. Clark will see patients by appointment at both the Braselton and Barrow hospital campuses – in suite 120 at Medical Plaza 1 and in the Imaging suite at NGMC Barrow.

Cancer navigation services are provided Monday through Friday, free of charge, either by physician or self-referral. Our navigators work to improve the patient experience by:

  • Assisting patients in coordination of care, often from diagnosis to completion of treatment and survivorship;
  • Assessing barriers to care and helping engage patients and family members in decision-making;
  • Educating patients and caregivers about the disease;
  • Empowering patients to communicate their healthcare treatment choices and actively participate in their care through self-management;
  • Eencouraging active communication between patients and healthcare providers; and
  • Providing access to financial resources and other support services.
